Full Mouth Dental Rehabilitation Of A Child Affected With Severe-Early Childhood Caries (S-ECC) Under General Anesthesia

The Department of Pediatric & Preventive Dentistry of Teerthanker Mahaveer Dental College & Research Centre (TMDC&RC), performed an interesting case of full mouth dental rehabilitation under General Anesthesia (G.A) of a four-year-old girl child affected with severe early childhood caries who was very much apprehensive and highly uncooperative on the dental chair. All the dental procedures were performed by the team of Pediatric Dentists headed by Professor & Head, Dr Ramakrishna Yeluri, Professor Harsimran Kaur, Senior Lecturer Dr Rishika along with postgraduate students and interns of the department.

The four-year-old girl child was experiencing pain in the upper and lower primary teeth for the past 6 months. Upon examination, the child was presented with multiple pulpally involved primary teeth requiring extensive full mouth dental rehabilitation that included preventive, endodontic, restorative and esthetic work. Since the child was highly uncooperative on the dental chair, the case was posted for dental rehabilitation in single sitting under general anaesthesia after gaining consent from the parents.

Under general anaesthesia and nasotracheal intubation, all the primary teeth were rehabilitated quadrant wise that included multiple pulpotomies, pulpectomies, stainless steel crowns, post & core followed by placement of strip crowns. The treatment was ended with the application of topical fluoride gel on all the teeth. The surgery lasted more than 4.5 hours and the outcome was excellent without any complication. After the postoperative period, the child was put on rigorous oral hygiene measures during the maintenance phase.

This case highlights the scope of Pediatric Dentistry regarding the delivery of dental care for young children using a multidisciplinary approach involving medical fraternity in a hospital set up.

Teerthanker Mahaveer’s University’s Department of Pediatric & Preventive Dentistry (TMDC&RC) has been performing on such explicit cases with the help of their postgraduate students. Such cases treatments are one of its kinds in the Moradabad region where extensive rehabilitation under general anaesthesia is executed in one sitting.

The department always includes interns to actively participate and allows them to play a supportive role in the team for each and every case for better learning and gaining motivation to choose this speciality as a postgraduate subject.
